Alive & Well Check
In-person or virtual meetings performed on a specified basis to document claimant’s health, medical treatment, next doctor’s visit, verification of employment, disability status, home environment, and physical restrictions. Frasco will schedule and conduct in-person or video meetings with claimants that warrant an alive & well check.
Ongoing Checks:
Frasco can conduct the alive & well checks on an as needed basis, on an annual schedule, semi-annual schedule or on a schedule predetermined by the client. When a client selects recurring alive & well checks, notice will be provided every 30 days listing upcoming checks for the next month.
Scheduled vs. Unannounced Checks:
On occasion, clients may request that checks be conducted without a pre-scheduled date and time.
Annual and Semi-Annual Checks:
Recurring visits allow Frasco to document the claimant’s general health, current medical treatment, current medications, the next scheduled doctor’s appointment, verification of employment, disability status, home environment, physical restrictions, hobbies, and activities over a period of time. The visits can be further customized according to client expectations and needs.
