Alive & Well Check

We conduct in-person or virtual meetings to document the claimant’s health, medical treatment, and next doctor’s visit. We also verify employment, disability status, home environment, and physical restrictions. Frasco will schedule and conduct Alive and Well Check or Wellness and Status Verification services through in-person or video meetings to ensure accurate claimant wellness verification and status updates.

Ongoing Checks:

Frasco can conduct the alive & well checks on an as needed basis, on an annual schedule, semi-annual schedule or on a schedule predetermined by the client. When a client selects recurring alive & well checks, notice will be provided every 30 days listing upcoming checks for the next month.

Scheduled vs. Unannounced Checks:

On occasion, clients may request that checks be conducted without a pre-scheduled date and time.

Annual and Semi-Annual Checks:

Recurring visits allow Frasco to document the claimant’s general health, current medical treatment, current medications, the next scheduled doctor’s appointment, verification of employment, disability status, home environment, physical restrictions, hobbies, and activities over a period of time. The visits can be further customized according to client expectations and needs.
